Schedules
Termination of a continuing care arrangement or capitation arrangement
- Terminate a continuing care arrangement with proper notice and completion of treatment
Referral to another dentist or to a hospital or other service
- Refer patient to another dentist or hospital if unable to provide required care
Treatment on referral
- Carry out prescribed steps when accepting a patient on referral
Inability of dentist to complete care and treatment under a continuing care arrangement or a capitation arrangement or treatment on referral
- Notify Agency if you cannot finish a patient's dental treatment
Care and treatment summaries
- Provide care and treatment summary to departing patients
Mixing of general dental services and private care and treatment
- You must not mislead patients to push them into private treatment
Occasional treatment
- Notify the Agency if you cannot complete occasional dental treatment
Referral for occasional treatment
- Refer patients to another dentist when you lack facilities or expertise
Remuneration
- Do not claim or accept payment for GDS treatment unless permitted
Standards of care
- Provide proper dental care and treatment with skill and suitable materials
General anaesthesia and sedation
- Arrange safe staffing for dental procedures using general anaesthesia or sedation
Issue of prescription forms
- Issue a signed prescription form to each patient for listed drugs
Domiciliary visits
- Visit and treat patients at home within 5 miles of practice
Records
- Keep patient records, retain for 2 years, and produce on request within 14 days
Prior approval of treatment
- Get prior approval before starting certain dental treatments
Completion of an estimate
- Provide full patient and treatment details on dental estimates
Completion of care and treatment under a continuing care arrangement or a capitation arrangement or treatment on referral
- Complete dental care and treatment within a reasonable time
Dental auxiliaries
- Ensure proper completion of care by dental auxiliaries
General dental services
- Provide dental care under the correct arrangement for each patient
Postgraduate education
- Maintain up-to-date dental knowledge and skills through CPD
Notices
- Display a notice of charges and exemption entitlement at practice premises
Patient information leaflets
- Provide a patient information leaflet about your dental services
Premises etc
- Provide suitable surgery, waiting room, equipment and admit inspectors
Deputies and assistants
- Manage deputies, assistants, and absences with Board notifications
Dentists suspended under section 28 or 32 of the Dentists Act 1984
- Ensure substitute dentist is on the dental list during suspension
Attendance
- Attend agreed health premises at agreed times and only provide general dental services
A continuing care arrangement
- Fulfil obligations under a continuing care arrangement
A capitation arrangement
- Examine, chart, inform, plan, and complete treatment for capitation patients
Emergency cover
- Make arrangements for emergency dental care under continuing care or capitation arrangements
Repair or replacement of restorations
- Repair or replace failed restorations free within 12 months
Duration and extension of a continuing care arrangement
- Ensure continuing care arrangements lapse after 2 years unless extended

- Examine patient and send details to Agency when extending capitation arrangement
