Application for inclusion in a performers list

Consideration of applications
Other duties (1) — Crown / regulator
- Local Health Board must verify applicant practitioners and check fraud/conduct records Local authority

- Notify Local Health Board of criminal/investigation events within 7 days Self-Employed Person
Conditions
- Notify Local Health Board of acceptance of conditions on initial inclusion and provide undertaking Self-Employed Person
